The Andhra Pradesh State Government does not have a single, dedicated, centralized online portal. The most reliable method is the offline (physical) application.
Determine the specific department (e.g., Revenue, APPSC). Find the Public Information Officer (PIO) name/address on the department's website.
Write a formal application on plain paper to the PIO. You can write in English or Telugu. Be precise about the documents you need.
Use an Indian Postal Order (IPO) or Demand Draft (DD) payable to the "Accounts Officer" of the authority. Cash is accepted if submitting in person.
Send via Registered Post (RPAD) or Speed Post. The tracking number is your legal proof of submission for future appeals.
Public authorities are legally required to publish this information suo motu (on their own). You shouldn't even need to file an RTI to get this data.

Procedure followed in the decision-making process, including channels of supervision.
The norms set by it for the discharge of its functions.
Rules, regulations, instructions, manuals, and records held/used by employees.
A statement of the categories of documents that are held by it.
Arrangements for consultation with the public in policy formulation.
Statement of boards, councils, committees, and other bodies constituted.
A directory of its officers and employees.
Monthly remuneration received by officers and employees.
Budget allocated to each agency, plans, expenditures, and disbursements.
Manner of execution of subsidy programs, amounts allocated, and beneficiaries.
Particulars of recipients of concessions, permits, or authorizations granted.
Details regarding information available to or held by it, reduced to electronic form.
Facilities available to citizens for obtaining info (e.g., library hours).
Names, designations, and other particulars of Public Information Officers.
Such other information as may be prescribed.
